NEW PATIENT INSTRUCTIONS:
1. Call my office at (408) 656-1576 to check for availability and schedule an initial appointment.
2. Fill out and submit the New Patient Intake Form ~ all information is encrypted and securely sent.
3. When you arrive at the office, please wait in the waiting room. I will come out to greet you at your appointed time.
